Section I. Funding Opportunity Description


1. Research Objectives

Purpose

This funding opportunity announcement (FOA), issued by the National Cancer Institute (NCI) and the National Institute of Dental and Craniofacial Research (NIDCR), encourages the submission of research grant applications from scientists who propose to study the factors that mediate initiation, use, and cessation of smokeless tobacco, to understand the relationship with other tobacco products, to develop methods for studying smokeless tobacco products and related behaviors in humans. The overall goal is to develop an evidence base to inform smokeless tobacco control efforts, and to develop effective ways to limit the spread and promote cessation of smokeless tobacco use.

Using the NIH Research Project Grant (R01) funding mechanism, this FOA focuses on discrete, specified, circumscribed projects based upon strong preliminary data.

Related Funding Opportunity: Investigators, who are interested in proposing early phase, pilot/exploratory, and/or hypothesis-driven projects, should submit applications in response to the partner FOA of identical scientific scope (RFA-CA-08-025), which uses the NIH exploratory/developmental (R21) grant mechanism.

Background

In the United States (U.S.), there are currently an estimated 8.2 million smokeless tobacco users older than 12 years of age. The use of smokeless tobacco has serious and potentially fatal health consequences for the user, and is known to cause oral and pancreatic cancers in humans. In addition, smokeless tobacco poses a particularly urgent health challenge because its use is persistently high among certain subgroups of the population and new tobacco products are being introduced and marketed to appeal to new users, including smokers who may be considering quitting.

At-Risk Populations. While the prevalence of smokeless tobacco use in the U.S. is relatively low (approximately 3% of adults and 8% of high school students), this aggregate statistic masks a much higher prevalence of use among certain population subgroups. The use of smokeless tobacco is higher among young white males, people living in southern and north central states, American Indians/Alaska Natives, people who are employed in blue-collar and service/laborer occupations, and those who are unemployed. A review of long-term trends in the use of smokeless tobacco in the U.S. found that in 2003, approximately 13% of 12th grade boys reported having used smokeless tobacco in the past 30 days; the prevalence was even higher among white high school boys in southern states. A survey of U.S. military personnel in 1994-95 found that approximately 24% of white men on active duty used smokeless tobacco. Unlike cigarette smoking, the use of smokeless tobacco is higher among athletes. For example, in 2003, approximately 25% of minor league and 36% of major league baseball players reported using smokeless tobacco. Additionally, 9% of American Indian/Alaska Native adults are current users of smokeless tobacco. Smokeless tobacco also poses a global threat to public health in some regions of the world, such as in India, smokeless tobacco use is the predominant form of tobacco use. Yet despite these statistics, little information is publicly available on the factors that contribute to such high prevalence among particular subpopulation groups.

New Marketing Trends for Tobacco Products. Recent trends indicate that the use of smokeless tobacco constitutes a growing challenge for public health. Marketing and sales of smokeless tobacco products have increased steadily over the past twenty years; for example, in 2005, the five largest tobacco manufacturers spent a record $251 million on smokeless tobacco advertising and other promotional efforts. Additionally, the use of smokeless tobacco has escalated in both youth and adult populations in recent years. Moreover, tobacco manufacturers have recently launched a new generation of novel smokeless tobacco products that may have even broader consumer appeal. In the past year, for example, major cigarette manufacturers Philip Morris and R.J. Reynolds have introduced new smokeless tobacco products using the familiar brand names Marlboro and Camel . Some of these new smokeless products use attractive flavorings, such as mint or fruit flavors, and new delivery methods, such as lozenges or small pouches that eliminate the need to spit. These product innovations may particularly appeal to youth and new users. New smokeless tobacco products are also being marketed to smokers for use in situations where they cannot smoke, which may adversely impact smoking cessation efforts.

Youth Initiation. The potential for initiation of tobacco use amongst youth poses a major, public health challenge. A recent study concluded that advertisements for smokeless tobacco products in magazines with substantial youth readership increased significantly between 1993 and 2002, despite efforts to control youth-targeted advertising during this period. Increased marketing and the introduction of new flavored products may increase initiation of smokeless tobacco use among youth. During the 1970s, the use of smokeless tobacco increased substantially among teens and young adults after the introduction of products that were more accessible to new users, with lower nicotine content and attractive flavorings. In addition, evidence suggests that users who begin with starter products, that are presumably low in nicotine, are more likely to subsequently graduate to products with higher nicotine content. Moreover, a number of studies suggest that the use of smokeless tobacco is associated with, and reinforces use of other tobacco products, including cigarettes. Thus, adolescents who use smokeless tobacco may also be more likely to progress to cigarette smoking.

Health Effects of Smokeless Tobacco Use. The serious health effects of smokeless tobacco have been well documented. A recent International Agency for Research on Cancer (IARC) review concluded that there is sufficient evidence, based on epidemiologic and laboratory studies, to conclude that smokeless tobacco causes oral cancer and pancreatic cancer in humans. At least 28 carcinogens have been identified in smokeless tobacco products, including tobacco-specific nitrosamines, which have been found to cause tumors of the nasal cavity, lung, trachea, pancreas, liver, and esophagus in animal models. Laboratory studies have uncovered biological pathways by which smokeless tobacco constituents cause cancer. Metabolites of the tobacco-specific nitrosamines have been observed in the urine of smokeless tobacco users in quantities as high as or higher than those observed in smokers. Moreover, hemoglobin adducts of carcinogens present in smokeless tobacco products are measurable in the blood of smokeless tobacco users, indicating that smokeless-tobacco-related carcinogens circulate throughout the body and may cause cancers outside the primary oral exposure route. The use of smokeless tobacco is also a cause of adverse oral health outcomes, including oral mucosal lesions, leukoplakia, and periodontal disease. Smokeless tobacco products contain nicotine, and users of smokeless tobacco products demonstrate signs of dependence similar to those of cigarette smokers, including tolerance with repeated use and symptoms of withdrawal upon cessation of use. Evidence suggests that dual users (i.e., who use both cigarettes and smokeless tobacco) demonstrate higher nicotine exposure levels and find cessation even more difficult to achieve compared with others who use only smokeless tobacco or cigarettes.

Research Needs. Current intervention strategies for cessation of smokeless tobacco use have had limited success. Clinical trials have shown that behavioral interventions in particular settings, such as cessation counseling in dental offices, may increase abstinence rates amongst users of smokeless tobacco. The available evidence is insufficient to support recommendations about the specific intervention components that should be applied. Additionally, trials of pharmacotherapies in users of smokeless tobacco, including the nicotine patch, nicotine gum, and bupropion, have shown no impact on long-term (i.e., longer than 6 months) rates of abstinence. Other independent studies suggest that pharmacotherapies may help reduce psychophysical symptoms associated with attempts at cessation, such as tobacco-related craving and weight gain, but without any clear long-term impact on cessation outcomes. Thus, further research is needed to better understand the initiation and progression of smokeless tobacco use and to develop novel cessation interventions.

Studying smokeless tobacco products and their use is complicated by the fact that a wide range of smokeless tobacco products are used in the U.S., including chewing tobacco, snuff, traditional oral tobacco products, such as iqmik, and novel products, such as tobacco lozenges. Yet, few data are publicly available on the properties of these products, how they are used, and their prevalence, within different population groups.

In addition to the known adverse biological effects of smokeless tobacco, the overall public health impact of smokeless tobacco use depends on a range of health and environmental factors, including the prevalence and patterns of use of different products in the population, the impact of marketing and advertising messages, and the effectiveness of prevention and cessation efforts. Because of a changing environment, including the introduction of new smokeless tobacco products, marketing to consumers who have not previously used smokeless tobacco products, and increasing restrictions on smoking in public places, there is an urgent need to support an organized research effort that will study changing patterns of smokeless tobacco use in a variety of populations. Additionally, because smokeless tobacco use may impact smoking initiation and/or cessation, a greater understanding is needed of the relationship between smokeless tobacco use and other tobacco use.

Specific Requirements, Objectives, and Scope
The purpose of this FOA is, through the resulting applications and associated projects, to build a strong scientific knowledge base on the use of smokeless tobacco use, particularly focused on measures and determinants of smokeless tobacco use prevention and cessation. This FOA will support laboratory, clinical, and population-based research, with an added focus on developing new methods for assessing smokeless tobacco use and exposure and studying determinants of its use, initiation, and cessation. Because the focus is on tobacco use behavior and patterns of use, it is expected that applicants responding to this FOA will propose to study smokeless tobacco products and their use in human subjects. It is also expected that the proposed research projects will emphasize observational studies. Innovative intervention studies may also be considered if they are focused on specific population subgroups with high prevalence of use of smokeless tobacco. Through this FOA, the NCI may also support studies that identify targets for population-based intervention by testing explanatory models of smokeless tobacco marketing and use. Results from laboratory studies of smokeless tobacco products have demonstrated relevance for understanding human tobacco use behavior. While the focus of this RFA is on smokeless tobacco use in the U.S., applications that contain proposals to conduct international studies are also appropriate if they have relevance to understanding the patterns of smokeless tobacco use in the U.S. To effectively address key research questions on the use of smokeless tobacco, applications should, where appropriate, incorporate multidisciplinary collaborations across the basic, behavioral, and clinical sciences, including epidemiology, psychology, biology, and chemistry.

Examples of pertinent research directions that may be addressed in response to this FOA include, but are not limited to, the questions raised below. 

The National Institute of Dental and Craniofacial Research (NIDCR) is particularly interested in applications that include oral health in explanatory models of smokeless tobacco initiation, use, or cessation, or those that study smokeless tobacco cessation in oral health clinics or contexts. Section II. Award Information


1. Mechanism of Support

This FOA will use the R01 award mechanism. The Project Director/Principal Investigator (PD/PI) will be solely responsible for planning, directing, and executing the proposed project.  

This FOA uses Just-in-Time information concepts (see SF424 (R&R) Application Guide). It also uses the modular as well as the non-modular budget formats (see http://grants.nih.gov/grants/funding/modular/modular.htm). Specifically, a U.S. organization submitting an application with direct costs in each year of $250,000 or less (excluding consortium Facilities and Administrative [F&A] costs) must use the PHS398 Modular Budget component.

U.S. applicants requesting more than $250,000 in annual direct costs and all Foreign applicants must complete and submit budget requests using the Research & Related (R&R) Budget component.

2. Funds Available

Because the nature and scope of the proposed research will vary from application to application, it is anticipated that the size and duration of each award will also vary. Although the financial plans of the NCI provide support for this program, awards pursuant to this funding opportunity are contingent upon the availability of funds.

F&A costs requested by consortium participants are not included in the direct cost limitation (see NOT-OD-05-004, November 2, 2004).

Any individual(s) with the skills, knowledge, and resources necessary to carry out the proposed research as the PD/PI is invited to work with his/her organization to develop an application for support. Individuals from underrepresented racial and ethnic groups as well as individuals with disabilities are always encouraged to apply for NIH support.

More than one PD/PI (i.e., multiple PDs/PIs), may be designated on the application for projects that require a team science approach and therefore clearly do not fit the single-PD/PI model.  Additional information on the implementation plans and policies and procedures to formally allow more than one PD/PI on individual research projects is available at http://grants.nih.gov/grants/multi_pi. All PDs/PIs must be registered in the NIH electronic Research Administration (eRA) Commons prior to the submission of the application (see http://era.nih.gov/ElectronicReceipt/preparing.htm for instructions).

The decision of whether to apply for a grant with a single PD/PI or multiple PDs/PIs grant is the responsibility of the investigators and applicant organizations and should be determined by the scientific goals of the project. Applications for grants with multiple PDs/PIs will require additional information, as outlined in the instructions below. The NIH review criteria for approach, investigators, and environment have been modified to accommodate applications involving either a single PD/PI or multiple PDs/PIs. When considering the multiple PD/PI option, please be aware that the structure and governance of the PD/PI leadership team as well as the knowledge, skills, and experience of the individual PDs/PIs will be factored into the assessment of the overall scientific merit of the application. Multiple PDs/PIs on a project share the authority and responsibility for leading and directing the project, intellectually and logistically.  Each PD/PI is responsible and accountable to the grantee organization, or, as appropriate, to a collaborating organization, for the proper conduct of the project or program, including the submission of required reports. This program does not require cost sharing as defined in the current NIH Grants Policy Statement.

3. Other-Special Eligibility Criteria

Applicants are not permitted to submit a resubmission application in response to this FOA.

SPECIAL INSTRUCTIONS

Applications with Multiple PDs/PIs

When multiple PDs/PIs are proposed, NIH requires one PD/PI to be designated as the "Contact PI, who will be responsible for all communication between the PDs/PIs and the NIH, for assembling the application materials outlined below, and for coordinating progress reports for the project. The contact PD/PI must meet all eligibility requirements for PD/PI status in the same way as other PDs/PIs, but has no other special roles or responsibilities within the project team beyond those mentioned above.

Information for the Contact PD/PI should be entered in Item 15 of the SF424 (R&R) Cover component.  All other PDs/PIs should be listed in the Research & Related Senior/Key Person component and assigned the project role of PD/PI.   Please remember that all PDs/PIs must be registered in the eRA Commons prior to application submission.  The Commons ID of each PD/PI must be included in the Credential field of the Research & Related Senior/Key Person component.  Failure to include this data field will cause the application to be rejected.

All projects proposing Multiple PDs/PIs will be required to include a new section describing the leadership plan approach for the proposed project.

Multiple PD/PI Leadership Plan: For applications designating multiple PDs/PIs, a new section of the research plan, entitled Multiple PD/PI Leadership Plan [Section 14 of the Research Plan Component in the SF424 (R&R)], must be included. A rationale for choosing a multiple PD/PI approach should be described. The governance and organizational structure of the leadership team and the research project should be described, and should include communication plans, process for making decisions on scientific direction, and procedures for resolving conflicts.  The roles and administrative, technical, and scientific responsibilities for the project or program should be delineated for the PDs/PIs and other collaborators.

If budget allocation is planned, the distribution of resources to specific components of the project or the individual PDs/PIs should be delineated in the Leadership Plan. In the event of an award, the requested allocations may be reflected in a footnote on the Notice of Award (NoA).

Applications Involving a Single Institution

When all PDs/PIs are within a single institution, follow the instructions contained in the SF424 (R&R) Application Guide.

Applications Involving Multiple Institutions

When multiple institutions are involved, one institution must be designated as the prime institution and funding for the other institution(s) must be requested via a subcontract to be administered by the prime institution. When submitting a detailed budget, the prime institution should submit its budget using the Research & Related (R&R) Budget component.  All other institutions should have their individual budgets attached separately to the Research & Related Subaward Budget Attachment(s) Form.  See Section 4.8 of the SF424 (R&R) Application Guide for further instruction regarding the use of the subaward budget form.

When submitting a modular budget, the prime institution completes the PHS398 Modular Budget component only.  Information concerning the consortium/subcontract budget is provided in the budget justification. Separate budgets for each consortium/subcontract grantee are not required when using the Modular budget format. See Section 5.4 of the Application Guide for further instruction regarding the use of the PHS398 Modular Budget component.

3.B. Submitting an Application Electronically to the NIH

Once an application package has been successfully submitted through Grants.gov, any errors have been addressed, and the assembled application has been created in the eRA Commons, the PD/PI and the Authorized Organization Representative/Signing Official (AOR/SO) have 2 weekdays (Monday Friday, excluding Federal holidays) to view the application image to determine if any further action is necessary.

Upon receipt, applications will be evaluated for completeness by the CSR and responsiveness by the NIH institute or center (IC). Incomplete and non-responsive applications will not be reviewed.

There will be an acknowledgement of receipt of applications from Grants.gov and the Commons. The submitting AOR/SO receives the Grants.gov acknowledgments. The AOR/SO and the PI receive Commons acknowledgments. Information related to the assignment of an application to a Scientific Review Group is also in the Commons.

Note: Since email can be unreliable, it is the responsibility of the applicant to check periodically on the application status in the Commons.

The NIH will not accept any application in response to this funding opportunity that is essentially the same as one currently pending initial review, unless the applicant withdraws the pending application. However, when a previously unfunded application, originally submitted as an investigator-initiated application, is to be submitted in response to a funding opportunity, it is to be prepared as a NEW application. That is, the application for the funding opportunity must not include an Introduction describing the changes and improvements made, and the text must not be marked to indicate the changes from the previous unfunded version of the application.

5. Funding Restrictions

All NIH awards are subject to the terms and conditions, cost principles, and other considerations described in the NIH Grants Policy Statement.

Pre-award costs are allowable. A grantee may, at its own risk and without NIH prior approval, incur obligations and expenditures to cover costs up to 90 days before the beginning date of the initial budget period of a new or renewal award if such costs: 1) are necessary to conduct the project; and 2) would be allowable under the grant, if awarded, without NIH prior approval. If specific expenditures would otherwise require prior approval, the grantee must obtain NIH approval before incurring the cost. NIH prior approval is required for any costs to be incurred more than 90 days before the beginning date of the initial budget period of a new or renewal award.

The incurrence of pre-award costs in anticipation of a competing or non-competing award imposes no obligation on NIH either to make the award or to increase the amount of the approved budget if an award is made for less than the amount anticipated and is inadequate to cover the pre-award costs incurred. NIH expects the grantee to be fully aware that pre-award costs result in borrowing against future support and that such borrowing must not impair the grantee's ability to accomplish the project objectives in the approved time frame or in any way adversely affect the conduct of the project (see the NIH Grants Policy Statement).

6. Other Submission Requirements and Information

During the funding period, all awardees of the RFA (R01 and R21) will be required to participate in a periodic (annual) joint meeting to be organized by NCI to share recent results and progress and address common challenges or methodological issues. Support for travel by the Principal Investigator and one co-investigator should be included in the proposed budget. For the purpose of estimating budgets, plan on a 1.5-day meeting each year, alternating between the Washington, DC, region and a location to be determined.

PD/PI Credential (e.g., Agency Login)

The NIH requires the PD(s)/PI(s) to fill in his/her Commons User ID in the PROFILE Project Director/Principal Investigator section, Credential log-in field of the Research & Related Senior/Key Person Profile component.

Sharing of Model Organisms:
NIH is committed to support efforts that encourage sharing of important research resources including the sharing of model organisms for biomedical research (see http://grants.nih.gov/grants/policy/model_organism/index.htm). At the same time, the NIH recognizes the rights of grantees and contractors to elect and retain title to subject inventions developed with Federal funding pursuant to the Bayh-Dole Act (see the NIH Grants Policy Statement). Access to Research Data through the Freedom of Information Act:
The Office of Management and Budget (OMB) Circular A-110 has been revised to provide access to research data through the Freedom of Information Act (FOIA) under some circumstances. Data that are: (1) first produced in a project that is supported in whole or in part with Federal funds; and (2) cited publicly and officially by a Federal agency in support of an action that has the force and effect of law (i.e., a regulation) may be accessed through FOIA. It is important for applicants to understand the basic scope of this amendment. NIH has provided guidance at http://grants.nih.gov/grants/policy/a110/a110_guidance_dec1999.htm. Applicants may wish to place data collected under this funding opportunity in a public archive, which can provide protections for the data and manage the distribution for an indefinite period of time. The periods of career award and LRP award may overlap providing the LRP recipient with the required commitment of time and effort, as LRP awardees must commit at least 50% of their time (at least 20 hours per week based on a 40 hour week) for 2 years to the research.
