Section I. Funding Opportunity Description

Objectives: The objective of this FOA is to support the development of human induced pluripotent stem (iPS) cells and other reprogrammed cells for aging and Alzheimer’s disease modeling.  The molecular and functional characterization of aged or Alzheimer’s disease phenotypes in specific cell types derived from human iPS and other reprogrammed cells is expected to provide proof-of-concept evidence that human reprogrammed cells can be valid cell models of aging and Alzheimer’s disease.

Background: Animal models of aging and disease have been critical to producing insights into disease mechanisms and for discovery and development of drugs for age-related disorders.  However, animal models are often imperfect at recapitulating the human disease pathophysiology or phenotype, probably due in part to differences in anatomy, biochemistry, genetics, and physiology between humans and other animals.   Some of the limitations of animal models potentially could be overcome by developing human cell models of aging and disease.  The discovery that human adult fibroblasts, and more recently other cell types, can be reprogrammed into iPS cells offers enormous potential for generating age-specific and disease-specific cells for analyses of aging and disease mechanisms in human cells.  In the near term, human iPS cells and their differentiated cell derivatives could be used for disease modeling to provide insight into disease mechanisms, including the effect of human genetic background on disease processes, and as platforms for drug screening and testing; more distantly, iPS cells might be used for cell and tissue replacement therapies.  Disease-specific iPS cell lines have been generated from individuals with a number of diseases, and increasingly disease-related phenotypes are being described in iPS-derived cells.

Recent studies have shown that cells can be reprogrammed directly into another cell type, without going through a pluripotent stage.  For example, fibroblasts have been reprogrammed into neural progenitor cells, neurons, or cardiomyocytes.  Such direct conversion of one cell type to another may generate cells that retain many epigenetic marks and more mature phenotypes than cells made from iPS cells.  Such trans-differentiated cells may offer an alternative to iPS cells in creating patient-specific cells for aging and disease modeling.

Modeling complex, late-onset neurodegenerative diseases, such as Alzheimer's disease (AD), in reprogrammed cells faces many challenges.  Foremost among them is the fact that the dynamics of disease progression over the lifespan of a patient likely will differ from the phenotypes developed over days or weeks in cells differentiated in vitro from patient-specific iPS cells.  Though cellular changes may occur decades before disease symptoms, special strategies may be needed to induce ‘aging’ in reprogrammed cells to bring out an age- or disease-specific phenotype. Aging is a major risk factor for Alzheimer’s and other diseases, so a better understanding of human cell aging is needed to fully appreciate the contribution of aging processes to age-related neurodegenerative and other diseases.  Reprogrammed cells from adult, elderly and centenarians, or from individuals with premature aging, like Hutchinson-Gilford Progeria Syndrome and Werner’s Syndrome, may provide extremely useful cell models of human aging to investigate the molecular and genetic mechanisms involved in healthy and abnormal aging.

Having human cells that better reflect or faithfully recapitulate a disease condition or aging processes in the most relevant cell type would enable: the detailed investigation of the genetic, molecular and cellular mechanisms underlying human aging and AD; the identification of positive and negative modifiers of the aging and AD phenotype via a functional genomics approach, for example by using siRNA; the linkage of genetic variants identified in GWAS and other genetic studies to molecular and biological phenotypes; and provide a platform for drug screening and toxicity testing. Such studies may lead to new mechanistic hypotheses on disease causation and therapeutic interventions.

Areas of Research: Reprogramming of human cells to model AD and aging processes holds great promise, but also has many unknowns.  To address those unknowns, exploratory research is needed to provide proof-of-concept evidence and to establish the most reliable and effective method for the efficient generation and differentiation of human reprogrammed cells that express aging- or AD-related phenotypes. Critical aspects include: the characterization -- at the genetic, epigenetic and functional level -- of age and/or disease related phenotypes in cells derived from human iPS and other reprogrammed cells; and the variability in iPS cell lines and differentiated cells derived from them.  Reproducibility between cell lines from the same individual and between different individuals should be considered in establishing the fidelity of reprogrammed cells as models of aging and AD. The influence of cell non-autonomous factors (e.g. paracrine or multi-cellular interactions) and the contribution of complex genetic and environmental factors in aging and sporadic diseases are additional challenges in using iPS cells to model aging and AD.  The tissue source of donor cells for making iPS cells also should be considered.  Research from this FOA should facilitate the development, characterization, and, possibly, standardization of cell reprogramming approaches to generate various cell types important in aging and AD. For cell models of human aging and AD, relevant cell types of interest include: brain neuronal, glial and microglial cells; heart and skeletal muscle cells; and cells of the pancreas and immune system.

Only studies generating iPS and other reprogrammed cells from human cells will be considered responsive to this FOA. Moreover, because working with reprogrammed cells is technically challenging, applicants (and/or collaborators) responding to this FOA should have relevant experience in cell reprogramming technology.  Such experience should be documented in the Biographical Sketch of the Program Director/Principal Investigator (PD/PI) and/or collaborators.

iPS and other reprogrammed cells developed under this FOA are considered unique research resources and as such NIH considers the sharing of such unique research resources an important means to enhance the value of NIH-sponsored research.   Reprogrammed cells developed under this FOA should be made readily available for research purposes to qualified individuals within the scientific community.  Applicants are required to comply with NIH policy on sharing research resources.  (see Section IV.2.).   

An annual meeting of the awardees from this FOA will be convened near the end of the first year or start of the second year of the award.  Funds for travel of the PD/PIto the Washington, DC area to attend this meeting should be included in the budget.

The number of awards is contingent upon NIH appropriations, and the submission of a sufficient number of meritorious applications.

NIA intends to fund an estimate of 5-6 awards, corresponding to a total of $1,200,000 for fiscal year 2012. Future year amounts will depend on annual appropriations.

Budget proposals are limited to $275,000 direct costs over a 2-year project period, with no more than $200,000 in direct costs allowed in a single year.

The R21 exploratory/developmental grant supports investigation of novel scientific ideas or new model systems, tools, or technologies that have the potential for significant impact on biomedical or biobehavioral research. An R21 grant application need not have extensive background material or preliminary information. Accordingly, reviewers will focus their evaluation on the conceptual framework, the level of innovation, and the potential to significantly advance our knowledge or understanding. Appropriate justification for the proposed work can be provided through literature citations, data from other sources, or, when available, from investigator-generated data. Preliminary data are not required for R21 applications; however, they may be included if available.

Overall Impact

Reviewers will provide an overall impact/priority score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).

Scored Review Criteria

Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.

Significance

Does the project address an important problem or a critical barrier to progress in the field? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?  

Investigator(s)    

Are the PD/PIs, collaborators, and other researchers well suited to the project? If Early Stage Investigators or New Investigators, or in the early stages of independent careers, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? Protections for Human Subjects

For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.

For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials. Section VI. Award Administration Information

1. Award Notices

If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement.

A formal notification in the form of a Notice of Award (NoA) will be provided to the applicant organization for successful applications. The NoA signed by the grants management officer is the authorizing document and will be sent via email to the grantee’s business official.

Awardees must comply with any funding restrictions described in Section IV.5. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. Any costs incurred before receipt of the NoA are at the recipient's risk. These costs may be reimbursed only to the extent considered allowable pre-award costs.

3. Reporting

When multiple years are involved, awardees will be required to submit the Non-Competing Continuation Grant Progress Report (PHS 2590) annually and financial statements as required in the NIH Grants Policy Statement.

A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.
