Section I. Funding Opportunity Description


1. Research Objectives

Purpose

The National Institute on Alcohol Abuse and Alcoholism (NIAAA) and the National Institute on Drug Abuse (NIDA) solicits grant applications to examine mechanisms contributing to concurrent alcohol and nicotine use and dependence. Co-occurring alcohol and nicotine dependence is common. Research suggests that alcohol dependence and nicotine dependence have similar genetic, neurochemical and behavioral characteristics. It is not well understood, however, whether or how a common mechanism(s) underlies these phenomenological similarities that contributes to the comorbidity of alcohol and nicotine use and dependence. The purpose of this program announcement is to promote research to study neurobiological and behavioral mechanisms of dependence and how alcohol and nicotine use interact through these mechanisms to promote dependence. Such an understanding is essential to guide the development of better prevention and treatment strategies for alcohol and nicotine co-abuse.

Background

Alcohol and nicotine are the most commonly misused non-therapeutic drugs in the United States. Nearly 50 million Americans smoke cigarettes; almost 20 million Americans are alcohol dependent or regularly drink alcohol in harmful quantities. These are not mutually exclusive populations. People who drink are three times more likely than the general population to smoke, and tobacco dependent individuals are four times more likely than the general population to be alcohol dependent.

Human and animal studies suggest a shared genetic predisposition to develop alcohol and nicotine dependence. Identical twins are twice as likely as fraternal twins to be alcohol and nicotine dependent if the other twin is dependent. The Collaborative Study on the Genetics of Alcoholism (COGA) identified a region on chromosome 2 that appears to be involved in both alcohol dependence and habitual smoking. Rats genetically selected for alcohol preference, but having no prior history of alcohol exposure, displayed a higher preference for nicotine and were more likely to reinitiate lever pressing for nicotine infusions following a single nicotine injection as compared to alcohol-non-preferring rats. In addition to seeking further research on common genetic determinants of alcohol and nicotine dependence, we also seek proposals to examine how genetic influences contribute to the development of common intermediate traits (e.g., risk taking, novelty seeking, and sensitivity to stress) that predispose one to alcohol and nicotine dependence. Furthermore, studies of how these common intermediate phenotypes impact alcohol and nicotine dependence are encouraged.

Human and animal studies have found that alcohol and nicotine exert both synergistic and antagonistic effects on behavior which may contribute to their concurrent use. For example, preclinical studies have shown that nicotines stimulant effects may offset alcohols acute sedative effects and cognition-impairing effects. Thus, the cognitive enhancing effects of nicotine could be more reinforcing in alcoholic smokers as compared to those in non-alcoholic smokers. Similarly, cognitive deficits associated with nicotine withdrawal are reduced by a low dose of ethanol in rats. Early postnatal exposure to alcohol enhances nicotines analgesic and reinforcing effects in adolescent rats, suggesting that early exposure to alcohol influences the strength of subsequent reinforcing effects of nicotine in later developmental stages. In humans, alcohol pretreatment increases cigarette smoking and craving as well as self-reported subjective rewarding effects of nicotine. Chronic nicotine treatment increased alcohol consumption in rats, and nicotine administration during periods of alcohol deprivation increased subsequent alcohol self-administration. Consistent with these observations, a transdermal nicotine patch enhanced alcohols subjective rewarding and pharmacological effects in humans. The mechanism underlying such effects, and whether the combination of alcohol and nicotine has a greater effect than either drug alone have not been thoroughly investigated.

Alcohol and nicotine may enhance a persons motivation to use either of the drugs by activating common brain targets that are responsible for the reinforcing effects of both drugs. One of the most probable targets for alcohol and nicotine to interact is nicotinic acetylcholine receptors (nAChRs). Several different types of nAChRs are involved in nicotine addiction. For example, cigarette smoking resulted in nearly complete occupancy of alpha4/beta2 nAChRs and was accompanied by a reduction in cigarette craving. This indicates that nicotines reinforcing effects may be mediated through alpha4/beta2 subunit-containing nAChRs. Alcohol enhances alpha4/beta2 nAChR function, but inhibits the activity of alpha7 nAChRs in vitro. By potentiating or inhibiting the function of different nAChRs, alcohol affects nicotines affinity for these receptors. Nevertheless, it remains to be determined whether and how nicotine and alcohol interactions at nAChRs contribute to their co-use. Both chronic alcohol and nicotine treatment increase nAChR expression in rodent and human brain tissue, providing additional evidence for modulation of nAChRs by these two drugs. However, the expression of different nAChR subtypes following chronic treatment with alcohol or nicotine alone or in combination has not been examined.

Studies have identified variations in the genes encoding the nAChR subunits which may account for differences in sensitivity to alcohol and nicotine. For example, mice expressing different forms of the alpha4 nAChR subunit differ in their nicotine preference when given a choice between water containing nicotine and tap water. Whether this alpha4 nAChR polymorphism modulates differential responses to some of the effects of alcohol remains to be determined. Further, alpha7 nAChR knock-out mice, but not beta2, beta3 or beta4 knock-out mice, were less sensitive to ethanol-induced memory impairments in contextual learning as compared to their wild type counterparts. Conversely, these alpha7 mutant mice did not respond to the cognitive enhancing effects of nicotine. This suggests the involvement of alpha7 gene in mediating alcohols ability to disrupt contextual learning. Future studies need to determine a role of these and other naturally occurring nAChR subtypes in modulating and/or mediating alcohol and nicotine use.

Nicotine and alcohol ultimately affect common neural circuits that support motivational and hedonic processes. Both drugs exert their reinforcing effects through complex interactions with many neurotransmitters including gamma-aminobutyric acid (GABA), glutamate, dopamine (DA), serotonin, and acetylcholine. This opens the range of receptor systems through which alcohol and nicotine interact to enhance the likelihood of their ingestion. Drugs that block nAChRs reduce alcohol drinking suggesting that nicotines primary receptor target plays a role in supporting alcohol ingestion. Pharmacological blockade of nAChRs in the ventral tegmental area (VTA) and nucleus accumbens attenuates alcohol consumption in rats. Varenicline, an alpha4/beta2 nAChR partial agonist, reduces alcohol drinking without affecting sucrose intake in rats. These interactions may be mediated by mesolimbic dopaminergic function. Blocking nAChRs in the VTA abolished alcohol-induced increase in dopamine release. nAChRs with alpha-conotoxin MII (alpha-CtxMII) binding sites are also involved in modulating alcohols behavioral effects. Administering alpha-CtxMII into the VTA reduces alcohol-induced release of DA, locomotor activity, and alcohol-induced conditioned reinforcement.

Alcohol also activates endogenous opioid pathways by stimulating mu or delta opioid receptors located on GABA interneurons leading to disinhibition of dopaminergic neurons and increased dopamine release in the nucleus accumbens. Conversely, stimulation of kappa opioid receptors by dynorphin decreases dopamine release and may account for the aversive effects of alcohol. Endogenous opioids may also play a role in the reinforcing effects of nicotine as nicotine has been reported to increase tissue levels of opioid peptides in the nucleus accumbens. Naltrexone, an opioid receptor antagonist approved for treating alcohol dependence, decreases the number of cigarettes smoked daily by human volunteers as well as self-reported satisfaction experienced during smoking. These findings suggest a role for endogenous opioid systems in maintenance of smoking and drinking. Additional research is needed to further delineate specific opioid-related processes contributing to alcohol and nicotine co-use and dependence.

In addition to endogenous opioid pathways, glutamate and cannabinoid receptor systems contribute to alcohol and nicotine dependence. Chronic alcohol and nicotine exposure, during development or adulthood, alters both ionic and metabotropic glutamate receptor mediated responses. However, very few studies have focused on the effects of the combined action of alcohol and nicotine on the glutamatergic system. It remains unclear whether and how the alteration of the glutamatergic system by either alcohol or nicotine contributes to their co-dependence. Rimonabant, a CB1 cannabinoid receptor antagonist, decreases alcohol consumption and blocked nicotine-induced conditioned place preference and suppressed nicotine-induced relapse to alcohol. This raises the need to further examine roles of glutamatergic and endocannabinoid systems in mediating concurrent alcohol and nicotine dependence.

Chronic exposure to alcohol or nicotine also regulates various transcriptional factors that are involved in drug abuse. For example, repeated alcohol or nicotine exposure reduces CREB phosphorylation but increases delta FosB expression in the nucleus accumbens. Given the importance of CREB and delta FosB in regulating target genes that mediate behavioral responses to drugs, future research should determine functional roles of these and other molecular adaptations to alcohol and nicotine co-abuse.

In addition to working on a common target(s), cross-tolerance is another biological factor that may play a role in co-occurring alcohol and nicotine use. Both alcohol and nicotine induce hypothermia and bradycardia. Chronic administration of alcohol to mice induced tolerance to behavioral effects of alcohol as well as cross-tolerance to the hypothermic and bradycardiac effects of nicotine, and vice versa. Thus, it is possible that following chronic exposure to nicotine, cross-tolerance develops to the intoxicating of alcohol, leading to an increase in alcohol intake. Moreover, following exposure to nicotine (or alcohol), cross-tolerance may develop to reinforcing effects of alcohol (or nicotine) resulting in an increased alcohol (or nicotine) use. This relationship may be especially important during adolescence in that adolescent nicotine exposure might facilitate alcohol use in adulthood. Further research examining nicotine and alcohol cross-tolerance across developmental stages is needed.

The above-mentioned neurobiological mechanisms may not be the only factors underlying concurrent alcohol and nicotine use. Cues associated with drugs can elicit feelings of craving and often trigger unintended relapse episodes in people trying to maintain abstinence. Alcohol and nicotine have well-defined stimulus properties. Thus, during alcohol and nicotine co-use, one drug may serve as a cue for the expectancy of the other. After abstaining from one drug, cues associated with ingesting the other, such as interoceptive discriminative stimulus effects, may precipitate craving and relapse. This possibility has received very little experimental attention, but may have important implications for designing cessation plans. Alternatively, in operant behavior studies, when two behaviors are consistently reinforced concurrently in the same environment, as is the case with concurrent smoking and drinking, discontinuing one reinforcer (e.g., smoking) can trigger the resurgence of behaviors directed toward the other reinforcer (e.g., drinking). Additional experimental research is needed to examine the consequences of discontinuing or administering one drug (i.e., either alcohol or nicotine) on reinstating the ingestion of the other.

Finally, as alcohol and nicotine use transitions from initiation to occasional, habitual and compulsive use, it is important to explore whether and how either of these drugs produce mutual transition to dependence. In rats, repeated cycles of alcohol intoxication and withdrawal promote neuroadaptations in many neural circuits leading to hyperexcitablity and to long-lasting changes in measures of stress and anxiety once alcohol administration is discontinued. Dysregulation of CRF neurons in the central nucleus of the amygdala mediates elevated drinking in post-dependent animals. Similarly, intermittent exposure to unlimited access to nicotine produced large and rapid intake of nicotine in rats. Many of the underlying neuroadaptive changes in response to chronic nicotine exposure remain unclear. It was recently shown that anxiety-like symptoms of withdrawal and excessive nicotine intake observed after abstinence occur through recruitment of the CRF system and activation of CRF1 receptors in the central nucleus of the amygdala. This raises the possibility that chronic alcohol exposure and chronic nicotine exposure each disrupt extrahypothalamic stress systems in the way that is predicted to generate dysphoria in absence of the drug and elevate the ingestion of either drug. Given that neuroadaptation is a key element in development of drug dependence and vulnerability to relapse, its role in conjoint alcohol and nicotine abuse needs to be examined further. This includes examining systems contributing to neuroplasticity as well other neuropeptides participating in post-dependent stress and anxiety responses (e.g., neuropeptide Y, nociceptin).

A complete understanding of alcohol and nicotine dependence will require a multifactorial approach that encompasses cellular, molecular, pharmacological, and behavioral aspects of their interactions. The knowledge gained from this initiative will provide the foundation for identification of brain circuits, molecular and behavioral mechanisms mediating alcohol and nicotine dependence. Such an understanding should lead to more effective prevention and treatment strategies for alcohol and nicotine co-abuse.

The evolution and vitality of the biomedical sciences require a constant infusion of new ideas, techniques, and points of view. These may differ substantially from current thinking or practice and may not yet be supported by substantial preliminary data. By using the R21 mechanism, the NIH seeks to foster the introduction of novel scientific ideas, model systems, tools, agents, targets, and technologies that have the potential to substantially advance biomedical research.

The R21 mechanism is intended to encourage new exploratory and developmental research projects. For example, such projects could assess the feasibility of a novel area of investigation or a new experimental system that has the potential to enhance health-related research. Another example could include the unique and innovative use of an existing methodology to explore a new scientific area. These studies may involve considerable risk but may lead to a breakthrough in a particular area, or to the development of novel techniques, agents, methodologies, models, or applications that could have a major impact on a field of biomedical, behavioral, or clinical research.

Applications for R21 awards should describe projects distinct from those supported through the traditional R01 mechanism. For example, long-term projects, or projects designed to increase knowledge in a well-established area, will not be considered for R21 awards. Applications submitted under this mechanism should be exploratory and novel. These studies should break new ground or extend previous discoveries toward new directions or applications.

Section II. Award Information


1. Mechanism of Support

This FOA will use the NIH Exploratory/Developmental Grant (R21) award mechanism. The Project Director/Principal Investigator (PD/PI) will be solely responsible for planning, directing, and executing the proposed project.  

This FOA uses Just-in-Time information concepts (see SF424 (R&R) Application Guide). It also uses the modular as well as the non-modular budget formats (see http://grants.nih.gov/grants/funding/modular/modular.htm). Specifically, a U.S. organization submitting an application with direct costs in each year of $250,000 or less (excluding consortium Facilities and Administrative [F&A] costs) should use the PHS398 Modular Budget component.

U.S. applicants requesting more than $250,000 in annual direct costs and all foreign applicants must complete and submit budget requests using the Research & Related Budget component.

2. Funds Available

Because the nature and scope of the proposed research will vary from application to application, it is anticipated that the size and duration of each award will also vary. Although the financial plans of the IC(s) provide support for this program, awards pursuant to this funding opportunity are contingent upon the availability of funds.

Facilities and Administrative (F&A) costs requested by consortium participants are not included in the direct cost limitation, see NOT-OD-05-004.

The total project period for an application submitted in response to this funding opportunity may not exceed 2 years. Although the size of the award may vary with the scope of research proposed, it is expected that applications will stay within the budgetary guidelines for an exploratory/developmental project; direct costs are limited to $275,000 over an R21 two-year period, with no more than $200,000 in direct costs allowed in any single year. Applicants may request direct costs in $25,000 modules, up to the total direct costs limitation of $275,000 for the combined two-year award period.

The decision of whether to apply for a grant with a single PD/PI or multiple PDs/PIs grant is the responsibility of the investigators and applicant organizations and should be determined by the scientific goals of the project. Multiple PD/PI Leadership Plan: For applications designating multiple PDs/PIs, a new section of the research plan, entitled Multiple PD/PI Leadership Plan, must be included. A rationale for choosing a multiple PD/PI approach should be described. The governance and organizational structure of the leadership team and the research project should be described, and should include communication plans, process for making decisions on scientific direction, and procedures for resolving conflicts. The roles and administrative, technical, and scientific responsibilities for the project or program should be delineated for the PDs/PIs and other collaborators.

If budget allocation is planned, the distribution of resources to specific components of the project or the individual PDs/PIs should be delineated in the Leadership Plan. In the event of an award, the requested allocations may be reflected in a footnote on the Notice of Award (NoA).

Applications Involving a Single Institution

When all PDs/PIs are within a single institution, follow the instructions contained in the SF424 (R&R) Application Guide.

Applications Involving Multiple Institutions 

When multiple institutions are involved, one institution must be designated as the prime institution and funding for the other institution(s) must be requested via a subcontract to be administered by the prime institution. When submitting a detailed budget, the prime institution should submit its budget using the Research & Related Budget component. All other institutions should have their individual budgets attached separately to the Research & Related Subaward Budget Attachment(s) Form. See Section 4.8 of the SF424 (R&R) Application Guide for further instruction regarding the use of the subaward budget form.

5. Funding Restrictions

Pre-award costs are allowable. A grantee may, at its own risk and without NIH prior approval, incur obligations and expenditures to cover costs up to 90 days before the beginning date of the initial budget period of a new or renewal award if such costs: 1) are necessary to conduct the project, and 2) would be allowable under the grant, if awarded, without NIH prior approval. If specific expenditures would otherwise require prior approval, the grantee must obtain NIH approval before incurring the cost. NIH prior approval is required for any costs to be incurred more than 90 days before the beginning date of the initial budget period of a new or renewal award.

The incurrence of pre-award costs in anticipation of a competing or non-competing award imposes no obligation on NIH either to make the award or to increase the amount of the approved budget if an award is made for less than the amount anticipated and is inadequate to cover the pre-award costs incurred. NIH expects the grantee to be fully aware that pre-award costs result in borrowing against future support and that such borrowing must not impair the grantee's ability to accomplish the project objectives in the approved time frame or in any way adversely affect the conduct of the project. Overall Impact. Reviewers will provide an overall impact/priority score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following five core review criteria, and additional review criteria (as applicable for the project proposed). 

Core Review Criteria.  Reviewers will consider each of the five review criteria below in the determination of scientific and technical merit, and give a separate score for each.  An application does not need to be strong in all categories to be judged likely to have major scientific impact.  For example, a project that by its nature is not innovative may be essential to advance a field.

Significance.  Does the project address an important problem or a critical barrier to progress in the field?  If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved?  How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?

Investigator(s).  Are the PD/PIs, collaborators, and other researchers well suited to the project?  If Early Stage Investigators or New Investigators, do they have appropriate experience and training?  If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)?  If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project?

Innovation.  Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions?  Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense?  Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed?

Approach.  Are the overall strategy, methodology, and analyses well-reasoned and appropriate to accomplish the specific aims of the project?  Are potential problems, alternative strategies, and benchmarks for success presented?   If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed?
If the project involves clinical research, are the plans for 1) Protections for Human Subjectss, and 2) inclusion of minorities and members of both sexes/genders, as well as the inclusion of children, justified in terms of the scientific goals and research strategy proposed?

Environment.  Will the scientific environment in which the work will be done contribute to the probability of success?  Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed?  Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements? 

Additional Review Criteria

As applicable for the project proposed, reviewers will consider the following additional items in the determination of scientific and technical merit, but will not give separate scores for these items.

Protections for Human Subjects. For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.

For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials.

Inclusion of Women, Minorities, and Children. When the proposed project involves clinical research, the committee will evaluate the proposed plans for inclusion of minorities and members of both genders, as well as the inclusion of children.

Vertebrate Animals. The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following five points: 1) proposed use of the animals, and species, strains, ages, sex, and numbers to be used; 2) justifications for the use of animals and for the appropriateness of the species and numbers proposed; 3) adequacy of veterinary care; 4) procedures for limiting discomfort, distress, pain and injury to that which is unavoidable in the conduct of scientifically sound research including the use of analgesic, anesthetic, and tranquilizing drugs and/or comfortable restraining devices; and 5) methods of euthanasia and reason for selection if not consistent with the AVMA Guidelines on Euthanasia.

Resubmission Applications. When reviewing a Resubmission application (formerly called an amended application), the committee will evaluate the application as now presented, taking into consideration the responses to comments from the previous scientific review group and changes made to the project.

Renewal Applications. When reviewing a Renewal application (formerly called a competing continuation application), the committee will consider the progress made in the last funding period.

Revision Applications. When reviewing a Revision application (formerly called a competing supplement application), the committee will consider the appropriateness of the proposed expansion of the scope of the project. If the Revision application relates to a specific line of investigation presented in the original application that was not recommended for approval by the committee, then the committee will consider whether the responses to comments from the previous scientific review group are adequate and whether substantial changes are clearly evident.

Biohazards. Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.


Additional Review Considerations

As applicable for the project proposed, reviewers will address each of the following items, but will not give scores for these items and should not consider them in providing an overall impact/priority score.

Budget and Period Support. Reviewers will consider whether the budget and the requested period of support are fully justified and reasonable in relation to the proposed research.

Sharing of Model Organisms:
NIH is committed to support efforts that encourage sharing of important research resources including the sharing of model organisms for biomedical research (see http://grants.nih.gov/grants/policy/model_organism/index.htm). At the same time the NIH recognizes the rights of grantees and contractors to elect and retain title to subject inventions developed with Federal funding pursuant to the Bayh-Dole Act (see the NIH Grants Policy Statement. Access to Research Data through the Freedom of Information Act:
The Office of Management and Budget (OMB) Circular A-110 has been revised to provide access to research data through the Freedom of Information Act (FOIA) under some circumstances. Data that are: (1) first produced in a project that is supported in whole or in part with Federal funds; and (2) cited publicly and officially by a Federal agency in support of an action that has the force and effect of law (i.e., a regulation) may be accessed through FOIA. It is important for applicants to understand the basic scope of this amendment. NIH has provided guidance at http://grants.nih.gov/grants/policy/a110/a110_guidance_dec1999.htm. Applicants may wish to place data collected under this funding opportunity in a public archive, which can provide protections for the data and manage the distribution for an indefinite period of time. NIH Public Access Policy Requirement:
In accordance with the NIH Public Access Policy, investigators funded by the NIH must submit or have submitted for them to the National Library of Medicines PubMed Central (see http://www.pubmedcentral.nih.gov/), an electronic version of their final, peer-reviewed manuscripts upon acceptance for publication, to be made publicly available no later than 12 months after the official date of publication. Standards for Privacy of Individually Identifiable Health Information:
The Department of Health and Human Services (HHS) issued final modification to the "Standards for Privacy of Individually Identifiable Health Information", the "Privacy Rule", on August 14, 2002. The Privacy Rule is a federal regulation under the Health Insurance Portability and Accountability Act (HIPAA) of 1996 that governs the protection of individually identifiable health information, and is administered and enforced by the HHS Office for Civil Rights (OCR).

Decisions about applicability and implementation of the Privacy Rule reside with the researcher and his/her institution. The OCR website (http://www.hhs.gov/ocr/) provides information on the Privacy Rule, including a complete Regulation Text and a set of decision tools on "Am I a covered entity?" URLs in NIH Grant Applications or Appendices:
All applications and proposals for NIH funding must be self-contained within specified page limitations. For publications listed in the appendix and/or Progress report, Internet addresses (URLs) or PubMed Central (PMC) submission identification numbers must be used for publicly accessible on-line journal articles. Publicly accessible on-line journal articles or PMC articles/manuscripts accepted for publication that are directly relevant to the project may be included only as URLs or PMC submission identification numbers accompanying the full reference in either the Bibliography & References Cited section, the Progress Report Publication List section, or the Biographical Sketch section of the NIH grant application. A URL or PMC submission identification number citation may be repeated in each of these sections as appropriate. There is no limit to the number of URLs or PMC submission identification numbers that can be cited.

The PHS strongly encourages all grant recipients to provide a smoke-free workplace and discourage the use of all tobacco products. In addition, Public Law 103-227, the Pro-Children Act of 1994, prohibits smoking in certain facilities (or in some cases, any portion of a facility) in which regular or routine education, library, day care, health care, or early childhood development services are provided to children. This is consistent with the PHS mission to protect and advance the physical and mental health of the American people.

Loan Repayment Programs:
NIH encourages applications for educational loan repayment from qualified health professionals who have made a commitment to pursue a research career involving clinical, pediatric, contraception, infertility, and health disparities related areas. The LRP is an important component of NIH's efforts to recruit and retain the next generation of researchers by providing the means for developing a research career unfettered by the burden of student loan debt. Note that an NIH grant is not required for eligibility and concurrent career award and LRP applications are encouraged. The periods of career award and LRP award may overlap providing the LRP recipient with the required commitment of time and effort, as LRP awardees must commit at least 50% of their time (at least 20 hours per week based on a 40 hour week) for two years to the research.
