1. Research Objectives
This funding opportunity announcement (FOA), issued by the National Cancer Institute (NCI), the National Institute of Nursing Research (NINR), and the Office of Dietary Supplements (ODS), all of the National Institutes of Health (NIH), solicits applications that can use information on gene polymorphisms from the Human Genome and International HapMap Projects to examine the validity of dietary interventions as personalized strategies for cancer prevention. These studies should be focused on the incidence of cancer in persons who are genetically susceptible carriers of polymorphisms or haplotypes, with the aim of identifying individual responders from non-responders to dietary interventions that can alter cellular cancer processes. The overarching goal is to facilitate the development of individualized, targeted nutrition-focused prevention strategies that can reduce cancer risk in individuals carrying genetic variations that alter their susceptibility to cancer.
Using the NIH Exploratory/Developmental Grant (R21) funding mechanism, this FOA focuses on early and conceptual stages of research projects.
Related Funding Opportunity: Investigators, who are interested in proposing discrete, specified, circumscribed projects based upon strong preliminary data, should submit applications in response to the parallel FOA of identical scientific scope (PA-08-220), which uses the NIH Research Project Grant (R01) funding mechanism.
Predicting responses to cancer preventive dietary interventions of individuals harboring defined genetic variations. As early as 1964, the World Health Organization concluded that several common cancers could arise from voluntary and deliberate lifestyle and environmental-related factors, suggesting that some cancers are preventable. A wealth of epidemiological and preclinical evidence supports the biologic role of several bioactive food components in modifying cancer risk, cancer-related processes, and tumor behavior. A comprehensive examination of the commonly occurring genetic variations across individual populations has been critical in determining individual cancer risks. Functional single nucleotide polymorphisms (SNPs) and haplotype blocks are commonly occurring genetic variations that have proven to be useful indicators of disease susceptibility. Several epidemiologic studies have suggested that some of the interactions between genetic variants and dietary components can act as modifiers of cancer risk. These studies suggest the need to identify vulnerable populations for targeted dietary interventions. Accounting for these commonly occurring individual genetic variations may also assist in resolving some controversies regarding the role of various dietary components in reducing cancer risk in humans.
Individual genetic variations in functional SNPs and haplotype blocks appear to contribute to varied biologic responses to dietary interventions. Recent studies incorporating genomic findings from the Human Genome (http://www.genome.gov/10001772) and HapMap Projects (http://www.hapmap.org/) have revealed profound non-uniformity of human responses to bioactive food components in terms of modifying disease risks. Individual genetic differences are known to strengthen the mechanistic rationale attempting to explain the variability in human physiologic responses to bioactive food components. Thus, it is important to provide mechanistic support for the notion that specific genetic variations may be critical in predicting both beneficial and harmful effects of particular dietary interventions aimed at modifying individual cancer risks.
Bioactive food components and genes that determine human cancer risk. Several bioactive food components can modify transcription, expression, and regulation of genetic targets that are known to influence numerous cancer-related biologic processes in humans, such as the detoxification of carcinogens and the development of adenomas and hyperplastic polyps. Individual genetic variants may influence carcinogenesis at multiple levels -- some polymorphic variants can either decrease or increase cancer risks. Individuals carrying some genetic variants also appear responsive to the effects of some bioactive food components at dietary attainable levels, only exhibiting reductions in cancer risk with elevated consumption of a particular bioactive food component. For example, the vitamin D receptor (VDR), which acts as a nuclear transcription factor, transactivates genes coding for the enzymes of the cytochrome P450 3A (CYP3A) subfamily, thereby stimulating hepatic detoxification of both lithocholic acid (LCA), a secondary bile acid and an enteric carcinogen. These events regulate the insulin growth factor-1 (IGF-1) signaling pathway. In addition, the VDR gene FOKI polymorphism, which occurs at the first start codon in exon 2, produces two differing gene products with dramatically altered functionality (F fully functional vs. f less functional alleles), resulting in profound phenotypic effects on calcium homeostasis. The less functional f FOKI variant is three amino acids longer compared with the F-allele. This subtle difference in ff gene product length impairs VDR-DNA binding, altering binding of the LCA ligand in VDR FOKI ff carriers, which ultimately results in a reduction of CYP3A transcriptional activation necessary for carcinogen detoxification.
Beneficial effect of bioactive food components on genetic variants that modify cancer processes. Recent studies suggest that bioactive food components may influence commonly occurring genetic variants to decrease cancer risks in some genetically susceptible individuals. For example, studies have suggested an association between reduced calcium absorption and increased colorectal cancer risk in individuals carrying either the VDR FOKI Ff or ff allelic variants. Carriers of the VDR FOKI Ff or ff alleles, consuming low levels of dietary calcium, were at increased risk for colorectal cancer compared to those carrying the VDR FOKI FF genotype. Because calcium regulates formation of the active metabolite of vitamin D, it has been hypothesized that dietary calcium intake may modify the influence of the VDR polymorphic variants on colorectal cancer risk. However, the risk for colorectal cancer was reduced when individuals, carrying either the high-risk VDR FOKI Ff or ff allelic variants, increased their dietary calcium intake. These findings have suggested that individualized interventional approaches to cancer prevention may be beneficial in reducing cancer risks.
Detrimental effects of bioactive food components on genetic variants that modify cancer processes. A disproportionate (i.e., imbalanced) consumption of some bioactive food components may actually place in case of individual carriers of specific genetic variants at a higher risk for cancer. Prevailing evidence suggests that there is great diversity in the responses of these polymorphic individuals to dietary interventions. Genetic variations may actually increase disease susceptibility in polymorphic individuals by altering various cellular processes. For example, a genetic variant in the kinase domain of the transient receptor potential melastatin 7 (TRPM7), which is a ubiquitously expressed constitutively-active ion channel, affects not only Mg+2 homeostasis, but also alters intracellular Ca+2 fluxes that control several cancer-related processes such as cell apoptosis, differentiation, proliferation, and motility. Individuals carrying a single mutated A allele who consumed disproportionately high dietary levels of calcium (relative to magnesium) were at a significantly higher risk for adenomas and hyperplastic polyps than those who did not carry this polymorphism. Strikingly, carriers of two mutated alleles who consumed high levels of calcium and magnesium in their diets were at a 16-fold higher risk for subsequent development of colon cancer compared with carriers of the wild-type GG control variant who consumed the same high levels of calcium and low levels of magnesium in their diets.
Dose-response effect of bioactive food components on carriers of genetic variants associated with cancer risk. Bioactive food components may exert a dose-dependent influence in carriers of genetic variants that regulate cancer processes. For example, phytoestrogens appear to reduce prostate cancer risk in men carrying one or two allelic C mutations in the promoter region of the estrogen receptor-b gene. Because the level of estrogen receptor-b gene expression in the prostate epithelium declines when malignant transformation occurs, the protective effect of dietary phytoestrogens may involve both estrogenic and anti-estrogenic actions. Recent studies have reported potential dose response effects of phytoestrogens; however, there is no definitive evidence of any apparent benefit in men consuming widely varying dietary phytoestrogen levels. When accounting for differential response to phytoestrogens based on differing estrogen receptor-beta gene variants, only those men carrying one or two allelic C estrogen receptor-b mutations benefited from the high consumption of phytoestrogens. Surprisingly, men carrying the wild-type TT genotype were not protected, and did not benefit from dietary phytoestrogens at any of the observed intake levels, suggesting genetic variation may attribute to a null response in these genetically susceptible individuals.
Influence of bioactive food components on functionally related combinations of genetic variants. Related genetic variants may also provide valuable predictive insights in to individuals responses to bioactive food components. For example, previous studies have suggested a greater risk for colorectal cancer (CRC) in individuals who regularly (>5 times/week) consume red meat. Yet, genetic variations in several key CYP enzymes necessary for carcinogen metabolism may dramatically influence actual cancer risks. In one case-control cohort study, genetic variations occurring in six functionally related CYPs as single SNPs and as collective interactions across the six SNPs representing both the CYP1 and CYP2 subfamilies were investigated. The resulting combined CYP variant profile, which commonly occurred in colorectal cancer cases, appeared useful as potential predictors of the efficacy of dietary interventions on modifying CRC risk. Collectively, carriers of all six CYP variants were at approximately 43-fold greater risk for CRC, compared with wild-type genotypes, when normalized for similar high red meat dietary intake. In the same study, a combination approach using commonly occurring DNA-based biomarkers of enzymatic activity, such as the CYP2C9 biomarker, appeared useful as potential identifiers of susceptible individuals. Such strategies may help identify those individuals who are at higher risk for cancer, and who may benefit most from targeted dietary interventions.
This initiative is crosscutting in scope, addressing several strategic objectives outlined in The NCI Strategic Plan (http://strategicplan.nci.nih.gov/) for the nations investment in cancer research. This FOA specifically addresses the objectives to accelerate progress in the following areas: cancer prevention; understanding the causes and mechanisms of cancer; understanding the factors that influence cancer treatment outcomes; and reducing and eliminating cancer-related health disparities.
Specific Research Objectives
This FOA is designed to facilitate the development of individualized, targeted nutrition-focused prevention strategies that can reduce cancer risk in genetically susceptible humans. Bioactive food components in dietary interventions may include essential nutrients and non-nutrients found in foods. In addition, bioactive food components may be consumed as either part of whole foods or in supplemental form. This FOA will not support preclinical studies; however, the information generated from such investigations might serve as the basis for exploratory pilot clinical studies in humans.

1. Mechanism of Support
This FOA will use the NIH Exploratory/Developmental Research Grant (R21) award mechanism. The Project Director/Principal Investigator (PD/PI) will be solely responsible for planning, directing, and executing the proposed project.
This FOA uses Just-in-Time information concepts see SF424 (R&R) Application Guide). It also uses the modular as well as the non-modular budget formats (see the Modular Applications and Awards section of the NIH Grants Policy Statement
All foreign applicants must complete and submit budget requests using the Research & Related Budget component.
2. Funds Available
Because the nature and scope of the proposed research will vary from application to application, it is anticipated that the size and duration of each award will also vary. Although the financial plans of the IC(s) provide support for this program, awards pursuant to this funding opportunity are contingent upon the availability of funds.
The total project period for an application submitted in response to this funding opportunity may not exceed 2 years. Although the size of award may vary with the scope of research proposed, it is expected that applications will stay within the budgetary guidelines for an exploratory/developmental project; direct costs are limited to $275,000 over an R21 two-year period, with no more than $200,000 in direct costs allowed in any single year. Applicants may request direct costs in $25,000 modules, up to the total direct costs limitation of $275,000 for the combined two-year award period. NIH grants policies as described in the NOT-OD-05-004, November 2, 2004.
1.B. Eligible Individuals
Any individual(s) with the skills, knowledge, and resources necessary to carry out the proposed research as the PD/PI is invited to work with his/her organization to develop an application for support. Individuals from underrepresented racial and ethnic groups as well as individuals with disabilities are always encouraged to apply for NIH support.
More than one PD/PI (i.e., multiple PDs/PIs) may be designated on the application for projects that require a team science approach and therefore clearly do not fit the single-PD/PI model.Additional information on the implementation plans and policies and procedures to formally allow more than one PD/PI on individual research projects is available at http://grants.nih.gov/grants/multi_pi. The NIH will not accept any application in response to this FOA that is essentially the same as one currently pending initial merit review unless the applicant withdraws the pending application. The NIH will not accept any application that is essentially the same as one already reviewed. However, the NIH will accept a resubmission application, but such application must include an Introduction addressing the critique from the previous review.

5. Funding Restrictions
All NIH awards are subject to the terms and conditions, cost principles, and other considerations described in the NIH Grants Policy Statement.
Pre-award costs are allowable. A grantee may, at its own risk and without NIH prior approval, incur obligations and expenditures to cover costs up to 90 days before the beginning date of the initial budget period of a new award if such costs: are necessary to conduct the project, and would be allowable under the grant, if awarded, without NIH prior approval. If specific expenditures would otherwise require prior approval, the grantee must obtain NIH approval before incurring the cost. NIH prior approval is required for any costs to be incurred more than 90 days before the beginning date of the initial budget period of a new award.
The incurrence of pre-award costs in anticipation of a competing or non-competing award imposes no obligation on NIH either to make the award or to increase the amount of the approved budget if an award is made for less than the amount anticipated and is inadequate to cover the pre-award costs incurred. NIH expects the grantee to be fully aware that pre-award costs result in borrowing against future support and that such borrowing must not impair the grantee's ability to accomplish the project objectives in the approved time frame or in any way adversely affect the conduct of the project. The NIH R21 exploratory/developmental grant is a mechanism for supporting novel scientific ideas or new model systems, tools, or technologies that have the potential to significantly advance our knowledge or the status of health-related research.
Because the Research Strategy is limited to 6 pages, an exploratory/developmental grant application need not have extensive background material or preliminary information as one might normally expect in an R01 application. Accordingly, reviewers will focus their evaluation on the conceptual framework, the level of innovation, and the potential to significantly advance our knowledge or understanding. Reviewers will place less emphasis on methodological details and certain indicators traditionally used in evaluating the scientific merit of R01 applications, including supportive preliminary data. Appropriate justification for the proposed work can be provided through literature citations, data from other sources, or, when available, from investigator-generated data. Preliminary data are not required for R21 applications; however, they may be included if available.
The goals of NIH supported research are to advance our
understanding of biological systems, to improve the control of disease, and to
enhance health. In their written critiques, reviewers will be asked to comment
on each of the following criteria in order to judge the likelihood that the
proposed research will have a substantial impact on the pursuit of these goals.
Each of these criteria will be addressed and considered in assigning the
overall score, and weighted as appropriate for each application. Note that an
application does not need to be strong in all categories to be judged likely to
have major scientific impact and thus deserve a meritorious impact/priority score. For
example, an investigator may propose to carry out important work that by its
nature is not innovative but is essential to move a field forward.
Overall Impact. Reviewers will provide an overall impact/priority score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following five core review criteria, and additional review criteria (as applicable for the project proposed).
Core Review Criteria. Reviewers will consider each of the five review criteria below in the determination of scientific and technical merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.
Significance: Does the project address an important problem or a critical barrier to progress in the field? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?
Investigator(s): Are the PD/PIs, collaborators, and other researchers well suited to the project? If Early Stage Investigators or New Investigators, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project?
Innovation: Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed?
Approach: Are the overall strategy, methodology, and analyses well-reasoned and appropriate to accomplish the specific aims of the project? Are potential problems, alternative strategies, and benchmarks for success presented? If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed?
If the project involves clinical research, are the plans for 1) protection of human subjects from research risks, and 2) inclusion of minorities and members of both sexes/genders, as well as the inclusion of children, justified in terms of the scientific goals and research strategy proposed?
Environment: Will the scientific environment in which the work will be done contribute to the probability of success? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements?

As applicable for the project proposed, reviewers will consider the following additional items in the determination of scientific and technical merit, but will not give separate scores for these items.
Protections for Human Subjects. For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials.
Inclusion of Women, Minorities, and Children. When the proposed project involves clinical research, the committee will evaluate the proposed plans for inclusion of minorities and members of both genders, as well as the inclusion of children.
Vertebrate Animals. The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following five points: 1) proposed use of the animals, and species, strains, ages, sex, and numbers to be used; 2) justifications for the use of animals and for the appropriateness of the species and numbers proposed; 3) adequacy of veterinary care; 4) procedures for limiting discomfort, distress, pain and injury to that which is unavoidable in the conduct of scientifically sound research including the use of analgesic, anesthetic, and tranquilizing drugs and/or comfortable restraining devices; and 5) methods of euthanasia and reason for selection if not consistent with the AVMA Guidelines on Euthanasia.
Resubmission Applications. When reviewing a Resubmission application (formerly called an amended application), the committee will evaluate the application as now presented, taking into consideration the responses to comments from the previous scientific review group and changes made to the project.
Renewal Applications. When reviewing a Renewal application (formerly called a competing continuation application), the committee will consider the progress made in the last funding period.
Revision Applications. When reviewing a Revision application (formerly called a competing supplement application), the committee will consider the appropriateness of the proposed expansion of the scope of the project. If the Revision application relates to a specific line of investigation presented in the original application that was not recommended for approval by the committee, then the committee will consider whether the responses to comments from the previous scientific review group are adequate and whether substantial changes are clearly evident.
Biohazards. Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.
2.B. Additional Review
As applicable for the project proposed, reviewers will address each of the following items, but will not give scores for these items and should not consider them in providing an overall impact/priority score.
Budget and Period Support. Reviewers will consider whether the budget and the requested period of support are fully justified and reasonable in relation to the proposed research.
Select Agents Research. Reviewers will assess the information provided in this section of the application, including 1) the Select Agent(s) to be used in the proposed research, 2) the registration status of all entities where Select Agent(s) will be used, 3) the procedures that will be used to monitor possession use and transfer of Select Agent(s), and 4) plans for appropriate biosafety, biocontainment, and security of the Select Agent(s).
Applications from Foreign Organizations. Reviewers will assess whether the project presents special opportunities for furthering research programs through the use of unusual talent, resources, populations, or environmental conditions that exist in other countries and either are not readily available in the United States or augment existing U.S. resources.
Resource Sharing Plans. Monitoring should be commensurate with risk. Sharing of Model Organisms:
NIH is committed to support efforts that encourage sharing of important research resources including the sharing of model organisms for biomedical research (see http://grants.nih.gov/grants/policy/model_organism/index.htm). At the same time the NIH recognizes the rights of grantees and contractors to elect and retain title to subject inventions developed with Federal funding pursuant to the Bayh-Dole Act (see the NIH Grants Policy Statement. Beginning October 1, 2004, all investigators submitting an NIH application or contract proposal are expected to include in the application/proposal a description of a specific plan for sharing and distributing unique model organism research resources generated using NIH funding or state why such sharing is restricted or not possible. This will permit other researchers to benefit from the resources developed with public funding. The inclusion of a model organism sharing plan is not subject to a cost threshold in any year and is expected to be included in all applications where the development of model organisms is anticipated.
Access to Research Data through the Freedom of
The Office of Management and Budget (OMB) Circular A-110 has been revised to provide access to research data through the Freedom of Information Act (FOIA) under some circumstances. Data that are: (1) first produced in a project that is supported in whole or in part with Federal funds; and (2) cited publicly and officially by a Federal agency in support of an action that has the force and effect of law (i.e., a regulation) may be accessed through FOIA. It is important for applicants to understand the basic scope of this amendment. NIH has provided guidance at http://grants.nih.gov/grants/policy/a110/a110_guidance_dec1999.htm. Applicants may wish to place data collected under this funding opportunity in a public archive, which can provide protections for the data and manage the distribution for an indefinite period of time. If so, the application should include a description of the archiving plan in the study design and include information about this in the budget justification section of the application. In addition, applicants should think about how to structure informed consent statements and other human subjects procedures given the potential for wider use of data collected under this award.
Inclusion of Women, Minorities, and Children:
It is the policy of the NIH that women and members of minority groups and their sub-populations must be included in all NIH-supported clinical research projects unless a clear and compelling justification is provided indicating that inclusion is inappropriate with respect to the health of the subjects or the purpose of the research. This policy results from the NIH Revitalization Act of 1993 (Section 492B of Public Law 103-43). The policy continues to require for all NIH-defined Phase III clinical trials that: a) all applications or proposals and/or protocols must provide a description of plans to conduct analyses, as appropriate, to address differences by sex/gender and/or racial/ethnic groups, including subgroups if applicable; and b) investigators must report annual accrual and progress in conducting analyses, as appropriate, by sex/gender and/or racial/ethnic group differences.
Inclusion of Children as Participants in Clinical
The NIH maintains a policy that children (i.e., individuals under the age of 21) must be included in all clinical research, conducted or supported by the NIH, unless there are scientific and ethical reasons not to include them.

Required Education on the Protection of Human Subject
NIH policy requires education on the protection of human subject participants for all investigators submitting NIH applications for research involving human subjects and individuals designated as key personnel. Human Embryonic Stem Cells (hESC):

NIH Public Access Policy Requirement:
In accordance with the NIH Public Access Policy, investigators funded by the NIH must submit or have submitted for them to the National Library of Medicines PubMed Central (see http://www.pubmedcentral.nih.gov/), an electronic version of their final, peer-reviewed manuscripts upon acceptance for publication, to be made publicly available no later than 12 months after the official date of publication. Standards for Privacy of Individually Identifiable
The Department of Health and Human Services (HHS) issued final modification to the "Standards for Privacy of Individually Identifiable Health Information", the "Privacy Rule", on August 14, 2002. The Privacy Rule is a federal regulation under the Health Insurance Portability and Accountability Act (HIPAA) of 1996 that governs the protection of individually identifiable health information, and is administered and enforced by the HHS Office for Civil Rights (OCR).
Decisions about applicability and implementation of the Privacy Rule reside with the researcher and his/her institution. This program is described in the Catalog of Federal Domestic Assistance at http://www.cfda.gov/ and is not subject to the intergovernmental review requirements of Executive Order 12372. Awards are made under the authorization of Sections 301 and 405 of the Public Health Service Act as amended (42 USC 241 and 284) and under Federal Regulations 42 CFR Part 52 and 45 CFR Parts 74 and 92. All awards are subject to the terms and conditions, cost principles, and other considerations described in the NIH Grants Policy Statement.
The PHS strongly encourages all grant recipients to provide a smoke-free workplace and discourage the use of all tobacco products. In addition, Public Law 103-227, the Pro-Children Act of 1994, prohibits smoking in certain facilities (or in some cases, any portion of a facility) in which regular or routine education, library, day care, health care, or early childhood development services are provided to children. This is consistent with the PHS mission to protect and advance the physical and mental health of the American people.
Loan Repayment Programs:
NIH encourages applications for educational loan repayment from qualified health professionals who have made a commitment to pursue a research career involving clinical, pediatric, contraception, infertility, and health disparities related areas. The LRP is an important component of NIH's efforts to recruit and retain the next generation of researchers by providing the means for developing a research career unfettered by the burden of student loan debt. Note that an NIH grant is not required for eligibility and concurrent career award and LRP applications are encouraged. The periods of career award and LRP award may overlap providing the LRP recipient with the required commitment of time and effort, as LRP awardees must commit at least 50% of their time (at least 20 hours per week based on a 40 hour week) for two years to the research.
