The Agency for Healthcare Research and Quality - Center for Primary Care, Prevention, and Clinical Partnerships intends to publish a Request for Application (RFA) soliciting research demonstration projects that focus on demonstrating innovative uses of healthcare technology to improve the quality of management of persons with complex healthcare needs in ambulatory settings, particularly during high risk transitions in care.  Of particular interest will be applications that address linkages across service providers as well as those that focus on the affected individual as the locus of care management.  The RFA, the fifth in AHRQ’s Ambulatory Safety and Quality program, is expected to be published in December, 2007

The review criteria will emphasize the ability of proposed tools and processes to be implemented in a variety of settings, including small and rural practices in a cost effective manner. 

AHRQ intends to use a Research Demonstration (R18) grant mechanism and anticipates that, contingent on the availability of funds, $2 – 3 million will be committed to this effort in FY2008.  It is anticipated that 5 -7 awards will be made.  Projects may be funded for a period of up to three years.
